5. See how we provide Quality Service to members.
6. Read the Grow and Stay Healthy Guidelines to get to know what immunizations (shots) you and your children need and when you need to have them done.
7. Read Molina Healthcare’s monthly newsletters about how to take care of your health.
8. Visit our Staying Healthy area if you have health questions. It offers all sorts of helpful info and advice.
9. Call Member Services at (866) 472-4585 if you have any questions about your health plan.

To receive your reward, complete your health exams as soon as possible. Once you have completed your screening, complete the attestation on the Member Portal.

Have you registered to use the portal?

If you haven’t, please follow the steps below to register for the portal. If you have already registered, you may need to create a new password.

If you have never registered, here’s how: 

• Go to MyMolina.com
• Click on ‘Create an Account’
• Have your valid email address, enrollment state, member ID, and date of birth ready to go
• Create a username and password
• Use your valid email address so you can get a code to verify that it is you. You can also add your mobile phone as another option for verification
• Enter that code into your registration screen and you’re logged in

Other things to know about the portal:

Your registration information will work on the web version from your computer, iPad or phone browser. You can download the app version as well. Visit your Android or Apple app store to find the My Molina app or use your phone camera to scan this QR code for easy access to the portal. 

List of Available Rewards (limitations apply)

• $20 reward for completing annual wellness exam (ages 20+ yrs, once per calendar year)
• $20 reward for completing well care child/adolescent visit (ages 0-21 yrs)
• $20 reward for completing a Health Risk Assessment (all members, once per calendar year)
• $20 reward for completing Behavior Health outpatient follow-up within 7 days after discharge (ages 6+ yrs)
• $20 reward for completing BOTH diabetic retinal eye exam AND HbA1c blood test with results under 8% (ages 18-75, once per calendar year)
• $25 reward for completing Weight Loss Program (once per calendar year)
• $25 reward for completing Smoking Cessation Program (once per calendar year)
• $25 reward for completing Substance Abuse Program (once per calendar year)

Pregnancy Rewards (applicable to pregnant mothers)

• $25 reward for telling us your pregnant within 30 days
• $25 reward for completing a Health Risk Assessment
• $50 reward for completing a prenatal visit within the first trimester
• $25 reward for completing additional (up to 4) prenatal appointments (max $100)
• $50 reward for completing a postpartum visit within 12 weeks following delivery
• $25 reward for completing your baby’s first well child visit within 30 days of birth
• $10 reward for completing a dental visit during pregnancy
