Passport Advantage (HMO D-SNP)
Passport Advantage (HMO D-SNP) is a plan for people who qualify for Medicare and Medicaid.
You are eligible to join the Passport Advantage (HMO D-SNP) plan if:
- You reside in the Passport Advantage (HMO D-SNP) geographic service area
- and – you must you must have both Medicare Part A and Medicare Part B
- and – you must be entitled to Medicare cost sharing assistance under Medicaid
- – and – you are entitled to Medicaid
